Enroll Your Child Into The Program

Please download the Rider Application Package 2023 form, then print it out and fill it in and mail it to the office.

Once the office receives the application form, you will be notified that it was received.

An assessment will be conducted by one of our therapists as openings become available.

Once accepted in the program the family must agree and sign the rider policy for every year your child is in the program. (Prairie Strides Rider Policy 2023)
